MS. LEE: Good morning, Green Valley Family Clinic. This is Ms. Lee. How may I help you?
CARLOS: Good morning. I’d like to make an appointment for a checkup, please.
MS. LEE: Of course. Are you a new patient or have you been here before?
CARLOS: I’ve been there before. My name is Carlos Rivera.
MS. LEE: Thank you, Mr. Rivera. Let me pull up your file. Can you confirm your date of birth?
CARLOS: Sure, it’s March 14th, 1988.
MS. LEE: Got it. What kind of appointment do you need — just a general checkup?
CARLOS: Yes, just my annual physical. Nothing urgent.
MS. LEE: All right. Dr. Adams has openings next Tuesday at 9:30 in the morning or Thursday at 2:15 in the afternoon. Which works better for you?
CARLOS: Tuesday morning works better. I start work at noon.
MS. LEE: Perfect. I’ll book you for Tuesday, September 29th at 9:30 a.m. with Dr. Adams.
CARLOS: Great. Do I need to bring anything?
MS. LEE: Please bring your photo ID and your insurance card. And try to arrive about fifteen minutes early to fill out the forms.
CARLOS: Okay. Will I need to fast before the appointment? You know, no food or water?
MS. LEE: For a general physical, fasting isn’t required. But if the doctor orders blood work, we’ll schedule that separately.
CARLOS: Understood. Can I ask — is there parking at the clinic?
MS. LEE: Yes, there’s a free parking lot right behind the building.
CARLOS: Perfect. One more thing — what’s your cancellation policy, in case something comes up at work?
MS. LEE: We just ask that you call us at least 24 hours ahead if you need to cancel or reschedule.
CARLOS: That’s fair. Could you please repeat the date and time once more?
